Crest may collect and disclose non-public information from you, from persons other than the individual(s) proposed for coverage, and from former customers, including, but not limited to, personal information and personal financial information about you and other individuals proposed for coverage as outlined in the chart below (“Personal Information”). Personal Information does not include information that is publicly available from government sources or information that is deidentified or aggregated.

(4) Biometric information, including genetic, physiological, behavioral, and biological characteristics, or activity patterns used to extract a template or other identifier or identifying information, such as, fingerprints, faceprints, and voiceprints, iris or retina scans, keystroke, gait, or other physical patterns, and sleep, health, or exercise data.
(5) Internet or other similar network activity, including browsing history, search history, information on a consumer’s interaction with a website, application, or advertisement, the type of device you are using, your IP address, the pages you visit while navigating the Site, and how long you visit certain pages.
(6) Geolocation data, including physical location or movements.
(7) Sensory data, including audio, electronic, visual, thermal, olfactory, or similar information.
(8) Professional or employment-related information, including current or past job history or performance evaluations.
(9) Non-public education information (per the Family Educational Rights and Privacy Act (20 U.S.C. Section 1232g, 34 C.F.R. Part 99)), including education records directly related to a student maintained by an educational institution or party acting on its behalf, such as grades, transcripts, class lists, student schedules, student identification codes, student financial information, or student disciplinary records.
(10) Inferences drawn from other personal information, including profile reflecting a person’s preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities, and aptitudes.

The Personal Information we may collect about you may come from the following types of sources and/or investigative techniques:


  1. Person(s), including you, when you:
    1. request a quote;
    2. apply for insurance coverage in the process of completing an insurance application;
    3. through applications or other forms and records received by us (for example, we may ask you to provide Personal Information to us when you enroll for insurance coverage and when you purchase an insurance policy through us);
    4. transact insurance business with us or our affiliates (for example, we may ask you to provide Personal Information to us when you conduct insurance business with us or our affiliates through postal mail, telephonically, electronically or by other means);
    5. provide claims information in the event of an insurance claim; and/or
    6. interact with us on social media platforms such as Facebook and Twitter.

Your Privacy Rights


You have the right to request and gain access to your own Personal Information collected and recorded by us and to receive information on how we have used your Personal Information. Pursuant to these rights, you may submit a written request to us for access to your recorded Personal Information. The written request must reasonably describe the Personal Information sought and that we can reasonably locate and retrieve. We reserve the right to charge a reasonable fee to cover the costs incurred in providing a copy of your recorded Personal Information to you.


You also have the right to correct or supplement your Personal Information that you believe is inaccurate. Pursuant to these rights, you may submit a written request to us to correct, amend or delete any recorded Personal Information about you that is within our possession.
